The Central East Community Care Access Centre (Central East CCAC) announces the launch of Telehomecare in Central East with the Central East Local Health Integration Network (Central East LHIN) and the Ontario Telemedicine Network (OTN)
The Telehomecare Program is funded by the Central East LHIN and Ontario Telemedicine Network. As an integral part of Ontario’s Chronic Disease Management Framework, Telehomecare aligns with and supports other provincial health strategies including Health Links.
In the Central East LHIN region, this program supports the Vascular Aim Strategy, of “continuing to improve the vascular health of residents so they spend 25,000 more days at home in their communities by 2016”. Telehomecare will also contribute to the reduction of Emergency Room visits and hospital re-admissions.
In the initial implementation, the Telehomecare program will focus on the Health Link Communities of Haliburton/Kawartha Lakes, Northumberland and Durham. Referrals to Telehomecare can be made by patients, caregivers, Primary Care Providers (PCPs), Specialists or Allied Health Professionals.
The Ontario Telemedicine Network (OTN) will provide program design and oversight as well as implementation and support services. OTN will also manage the technology, maintain an expert clinical advisory committee and will certify clinicians to deliver Telehomecare.
How to Refer
Admission to the Central East LHIN Telehomecare Program is completed through the Central East CCAC. Patients and caregivers can call 1-844-607-2549.
Primary Care Providers can complete the Central East CCAC Referral/Request for Assessment form or the COPD & Heart Failure Telehomecare Referral Form
